TO: Any person having an interest in, or the care, custody or control of DIANA L. ST. JOHN (“Protected Person”).
PLEASE TAKE NOTE that SPRING VALLEY HOSPITAL, has filed a petition seeking for the above-named Court to name a guardian over the Protected Person and to declare the Protected Person incapacitated or of limited capacity. The rights of the Protected Person may be affected if the petition is approved. The Protected Person has a right to appear at the hearing scheduled on the petition to oppose it. The Protected Person also has the rights to be represented by an attorney, who may be appointed for the Protected Person by the above-named Court if the Protected Person is unable to retain one. YOU ARE HEREBY CITED AND NOTIFIED to appear before a Judge of this Court at the date, time and place specified below and to show cause, if any why DIANA L. ST. JOHN should not be declared incapacitated or otherwise in need of a guardian for the person and/or estate and to further show cause, if any, why KARYN SMITH, Nye County Public Guardian, should not be appointed to act as general guardian of the Protected Person’s person and/or estate. THIS CITATION is based upon the verified Petition for Appointment of Nye County Public Guardian as a general Guardian of the Person and Estate and for Issuance of Letters of Guardianship filed by Spring Valley Hospital. PLEASE NOTE: You do not need to appear unless you wish to oppose the guardianship and enter an objection.
